To receive information about listing your products on the CollectiveMed.com website, please complete the following form. We will respond with instructions for becoming an official CollectiveMed.com partner.

All resellers and manufactures are required to have their own website. A link will be placed in the "Accessories" section of the PDA Center. Your company will receive a "Details" page with your company logo, description of your company's products, contact information and a link to your website.

• Monthly Listing Fee: $15
• Yearly Listing Fee $150 (2 months free)
